Mechanical ventilation in severe asthma.

چکیده
Asthma is a cause of severe morbidity and mortality in the United States. Severe acute asthma accounts for over one million emergency room visits per year (1). The vast majority of patients presenting with acute asthma can be successfully managed with β2-agonist bronchodilators, corticosteroids, and oxygen. Despite maximal medical therapy, it is estimated that 1– 3% of patients with severe asthma may require endotracheal intubation and mechanical ventilation during their hospital course (2,3). For the purposes of this chapter, severe asthma requiring mechanical ventilatory support will be referred to as status asthmaticus (SA). It appears that the number of patients progressing to SA is increasing, especially in the United States (4,5).

Clinical evaluation of severe asthma attacks requiring tracheal intubation and mechanical ventilation.
BACKGROUND Although the number of patients requiring hospitalization due to asthma attacks has decreased over the years, there are many who still require hospitalization for tracheal intubation and mechanical ventilation following a severe asthma attack. Therefore, we evaluated the characteristics of patients with asthma who required tracheal intubation and mechanical ventilation in our hospita...
متن کاملClinical review: Mechanical ventilation in severe asthma
Respiratory failure from severe asthma is a potentially reversible, life-threatening condition. Poor outcome in this setting is frequently a result of the development of gas-trapping. This condition can arise in any mechanically ventilated patient, but those with severe airflow limitation have a predisposition. It is important that clinicians managing these types of patients understand that the...
متن کاملCharacteristics of patients with soybean dust-induced acute severe asthma requiring mechanical ventilation.
Patients presenting with acute severe asthma during the Barcelona's outbreaks caused by soybean dust inhalation from August, 1981, through September, 1987, characteristically showed an abrupt severe onset of each attack followed by a rapid relief of symptoms after treatment. To throw further light on clinical findings, pathophysiology and outcome in the most life-threatening episodes, we review...
متن کاملMechanical ventilation for severe asthma.
Acute exacerbations of asthma can lead to respiratory failure requiring ventilatory assistance. Noninvasive ventilation may prevent the need for endotracheal intubation in selected patients. For patients who are intubated and undergo mechanical ventilation, a strategy that prioritizes avoidance of ventilator-related complications over correction of hypercapnia was first proposed 30 years ago an...
